Termination of Coverage
Healthcare and Dental Plans
Your Healthcare and/or Dental Plan coverage stops at the end of the month following the month of your last premium deduction (i.e., after you end employment, retire, die, waive or are deemed to have waived participation during an unpaid leave of absence or layoff). For example, if your last premium deduction is made in June, your coverage will cease July 31. A premium will be deducted from your final pay if your last day worked falls in a regular deduction cycle at your employer.
You have a 90-day claim limitation period (CLP) from the end of the coverage termination date to submit claims for any eligible expenses incurred prior to termination, and benefit payments will be limited to your maximum benefit amount.
Healthcare Spending Account
The HSA benefit will stop at the same time your HEB Manitoba Healthcare Plan coverage stops, which is at the end of the month following the month of your last premium deduction (i.e., after you end employment, retire, die, waive or are deemed to have waived participation during an unpaid leave of absence or layoff).
You have a 90-day CLP from the end of the coverage termination date to submit claims for any eligible expenses incurred prior to termination, and benefit payments will be limited to your maximum benefit amount. Any of your HSA benefit amount remaining after the 90-day CLP will be forfeited.
