Submitting Your Claims
Claim Forms are available from HEB Manitoba, but submitted to Manitoba Blue Cross. Please ensure Claim Forms are properly completed. Incomplete forms will be returned for clarification/re-submission.
Receipts for small claims should be accumulated until they total at least $25. If less than $25, submit at the end of the calendar year.
All receipts/bills should be on the printed letterhead of the service provider and must show clearly:
- Name of the patient,
- Description of the service provided, and
- Date(s) of the service provided.
Claims must be submitted within two years of the date(s) services were provided.
